​Once your child has received a diagnosis, the next step is to submit our intake form (click here). This helps us learn about your child and get your family on file with us. It typically takes 15–20 minutes to complete and can be filled out on your phone or computer.

• Insurance card (front and back) Once your form is submitted, your family will be added to our waitlist.
